FINANCIAL POLICY
For your convenience we accept American Express, Care Credit, Cash, Check, Discover, Mastercard, and Visa. Payment is expected at the time services are performed. Balances outstanding for over 60 days will incur a monthly 1.5% finance charge which equals to 18% per annum rate. There is a $25 returned check fee. Refunds for overpayment will be sent after all treatment is completed and insurance has been collected.
APPOINTMENT POLICY
An appointment in our schedule is a bond of trust that we will be here to serve you and you will be present for treatment. We strive to create a schedule that most efficiently provides for the dental needs of all our patients. Please arrive on-time to your scheduled appointment. Late arrivals cause schedule delays for those patients who arrive promptly for their appointment time. If time allows, late arrivals will be worked into the schedule or re-appointed to another day. We respect our patients’ time and make every effort to remain on schedule. Some visits are more complicated than initially anticipated, and emergencies may arise that could delay us. If we are significantly delayed, every effort will be made to notify you beforehand so you may choose to come later or reschedule.
CANCELLATION POLICY
We understand that things may come up, it’s very important that we receive notice of a change in plans at least 48-hours in advance. Our practice is dedicated to quality care and our doctors and team spend an extensive amount of time preparing for your scheduled visit. Cancelled and missed appointments create lost treatment time for our other patients as well as lost production time for our doctors and staff. We ask that you make every effort to give us at least a 48-hour notice if you cannot make your scheduled appointment. It is our policy to charge a patient for a broken appointment. If a scheduled appointment is cancelled prior to 48 hours notice including no shows, a fee of $50.00 – $100.00 based on the length and type of appointment will be charged to your credit card on file.
